Features & Benefits
The scanned forms module provides an alternative data entry modality. Instead of entering the patient data directly on a computer, it can simply be filled in on a paper form. The forms are scanned and the data recognized and appended to the database.

Scanned Forms contain the following items:

  • Sets of scannable forms that cover vascular operations, endovascular interventions, hemodialysis access, and noninvasive lab studies
  • Copy of Cardiff Teleform (Designer, Reader and Verifier)
  • Compatible scanner with auto-document feeder

The advantages of Scanned Forms are:

  • The forms can be distributed anywhere
  • Filling a form has almost no learning curve.
  • Since the form is filled while the care is provided and filled by the provider, the quality of the data is much better than retrospective data entry.
  • The end users do not need a computer nor require a software license.
  • Save time and money.
  • Standardize the data entry.

The forms are filled, scanned, read, verified and its data appended to the corresponding application swiftly. You can scan, verify and append data of about 25 sheets in less than 10 minutes.

End users can modify the forms with some limitations. You can change the contents of existing fields, but you can not add or delete fields. However, we can customize the forms and any of our applications to meet your needs. (link to services, data scan).

The forms are typically filled where the care is provided, e.g., vascular lab, surgery office. You can easily add your logo to a form. Thus scanned forms can become part of the standard hospital forms to eliminate duplicate entry.
